Your insurance policy mandates that co-pays be paid at the
time of service. If the amount of your co-pay is uncertain,
a charge of
$30.00
will be collected during your visit to be applied toward
your co-pay. Additionally, you are required to cover any
portion of the service that applies to your insurance
deductible.

We accept cash, credit cards, debit cards, and checks. Accounts unpaid for over 90 days may lead to dismissal from the practice. If you're facing difficulties in making payments, please contact our billing specialist. Regarding insurance, we will submit your charges directly to your provider. If Dr. Ruoff is not part of your insurance network, you will be responsible for any balance remaining after your insurance contributes. It is your responsibility to understand your insurance policy and provide us with the latest insurance card for accurate billing. For auto accident visits, please contact your insurer and provide us with the claim number, billing details, adjuster's name, and contact information; otherwise, full payment will be required at the time of your visit.
You will receive bills and statements for services such as
hospital stays, lab tests, pathology, and radiology that are
requested through our office but provided by other
healthcare entities. Please direct any inquiries about their
billing and services to them directly.
The ideal time to request a medication refill is during your office visit. Please keep track of your medication levels to know when you are running low. Refill requests made by phone or through the patient portal take 24 to 48 hours to process and are handled during regular office hours. Due to a high volume of requests, prior authorizations for medications by your insurance may take up to 7 business days. In some cases, a visit may be necessary to reassess your medical condition before refilling your prescription.
Patients must sign a contract and adhere to its terms. To
ensure compliance, random urine tests may be conducted.
Referrals to pain specialists may be recommended. We may
also request reports from the state pharmacy database to
confirm that medications are used as prescribed. Please note
that refills are not available outside of regular office
hours, and on-call physicians will not issue refills for
these medications.
Please provide at least 24 hours' notice if you need to
cancel your appointment. Missing three appointments without
notice may lead to dismissal from the practice. If you
arrive more than 10 minutes late, you may be asked to
reschedule to prevent delays for other patients.
At your first visit, and periodically thereafter, you must
sign a HIPAA authorization. This ensures your privacy,
enables us to bill your insurance for services provided, and
allows communication with other healthcare providers
involved in your care.
